Death Certificates

In keeping with CDC guidelines, City Hall is open to the public with social distancing policies in place. Please be aware that there may be delays for visitors as there are restrictions on the number of people in City offices for safety reasons.


Obtaining a death certificate

Cash, money orders, or cashier's checks are accepted; no personal checks or credit cards. For your convenience you may submit copy requests by mail.  All mail requests are processed on the day they are received in our office. Please refer to the appropriate application form and identification requirements below.

Accessing Death Certificates

Any person may obtain a copy of a death certificate. Social Security Numbers WILL BE REMOVED as applicable, in accordance with Public Act 97-7. The charge is $20.00 for a death certificate, and all copies will be certified with a raised seal. If you are the next of kin/informant listed on the death certificate, please provide identification such as a driver’s license or state-issued ID.
